Named GP; all GP practices are now required by NHS England to give a named GP to their patients. If you wish to know who yours is then please ask Reception. Thank you.

Risk Register; If you have 1 or more long term conditions that place you at risk of an emergency hospital admission then you may be on our 'Risk Register'. Patients on our Risk Register have an annual review with their GP and a proactive Care Plan designed to try and reduce the risk of a hospital admission and help you to better manage your own health. You should have your Care Plan reviewed every year. Please ask if you would like to be involved.

We are no longer taking requests for repeat medication over the phone but you can order it yourself now via this website. If you do not have your login details for SystmOnline yet then ask at Reception. You can also book your own appointments this way.
